Maintenance Requests

Online Requests (Recommended)

Submit a maintenance request anytime through AppFolio:

     After logging in, click the "Maintenance" tab.

Online requests are the preferred method for all non-emergency maintenance issues.

Paper Maintenance Requests

If you prefer a paper form:

Preliminary Work Request Form – (Log into AppFolio to download the form to print.)

Printed forms are also available in the Clubhouse Library.

Completed forms may be placed in the Drop Box located in the Clubhouse Meeting Room next to the office or forms can be dropped off or mailed to Pfefferle at 200 E. Washington Street, Appleton.
